End-of-Life Guidance

We provide gentle guidance on what to expect during the final stages — including normal changes in eating, drinking, and sleeping patterns. You will never be alone in this process. Our nurses are available 24 hours a day to walk with you through every step.

Highly Qualified Staff - Westlake Village Hospice

Our staff at Westlake Village Hospice takes pride in serving patients throughout Los Angeles and Ventura County. Throughout a patient's time in Hospice, they will work with various members of our interdisciplinary team.

The Hospice Physician

Oversees the medical plan, certifies eligibility, prescribes comfort medications, and is available to see patients as often as needed to ease suffering.

Skilled Registered Nurses (RN)

Provide regular assessment visits and are available 24/7 for phone support and emergency home visits. They manage medications, wounds, and symptom changes.

Hospice Aides (CHHA)

Certified Home Health Aides help with personal care such as bathing, grooming, and dressing — preserving the patient's dignity and giving family caregivers a rest.

Medical Social Worker

Provides emotional counseling, connects families to community resources, assists with advance directives, and facilitates family communication around care goals.

Chaplain / Spiritual Counselor

Offers non-denominational spiritual support to patients and families of all faiths and backgrounds — helping find peace, meaning, and closure.

Trained Volunteers

Provide companionship, read aloud, run errands, and offer respite for family caregivers — giving primary caregivers time to rest, eat, and recharge.
