The National Hospice Palliative Care Organization defines Hospice as a team oriented approach to expert medical care, pain management, and emotional and spiritual support expressly tailored to the patient's needs and wishes. Support will be extended to the patient's loved ones as well.

Your Primary Care Physician and a Hospice Physician agree that the Hospice criteria for a terminal illness is present, usually with a prognosis of six months or less. Our Hospice team will perform a specialized, individualized assessment taking into consideration any specific care needs and tailor a care plan centering on personal choices and needs.
We provide more than just medical monitoring. Our team includes doctors, nurses, social workers, and spiritual counselors to support the whole person — body, mind, and spirit.
You can receive hospice care wherever you call home — a private residence, an assisted living facility, or a nursing home. We come to you.
Patients can choose to leave hospice care at any time if their condition improves or if they wish to seek curative treatment again. There is no penalty.

Our staff at Westlake Village Hospice takes pride in serving patients throughout Los Angeles and Ventura County. Throughout a patient's time in Hospice, they will work with various members of our interdisciplinary team.
Oversees the medical plan, certifies eligibility, prescribes comfort medications, and is available to see patients as often as needed to ease suffering.
Provide regular assessment visits and are available 24/7 for phone support and emergency home visits. They manage medications, wounds, and symptom changes.
Certified Home Health Aides help with personal care such as bathing, grooming, and dressing — preserving the patient's dignity and giving family caregivers a rest.
Provides emotional counseling, connects families to community resources, assists with advance directives, and facilitates family communication around care goals.
Offers non-denominational spiritual support to patients and families of all faiths and backgrounds — helping find peace, meaning, and closure.
