The National Hospice Palliative Care Organization defines Hospice as a team oriented approach to expert medical care, pain management, and emotional and spiritual support expressly tailored to the patient’s needs and wishes. Support will be extended to the patient’s loved ones as well.
At the center of Hospice is the belief that each of us has the right to die pain-free and with dignity and that our loved ones will receive the necessary support to allow us to do so. We provide home hospice care for patients in Los Angeles, Ventura County, Antelope Valley, and the San Fernando Valley.
Hospice is palliative / comfort care and is an option after all other medical treatments have been explored. The goal is to prevent and relieve suffering, to improve quality of life for individuals experiencing a serious, complex illness at the end of life.
Your Primary Care Physician and a Hospice Physician agree that the Hospice criteria for a terminal illness is present, usually with a prognosis of six months or less. Our Hospice team will perform a specialized, individualized assessment taking into consideration any specific care needs and tailor a care plan centering on personal choices and needs.
Our staff at Westlake Village Hospice takes pride in serving patients throughout Los Angeles and Ventura County. Throughout a patient’s time in Hospice, they will work with various members of our interdisciplinary team. We have registered nurses on staff to provide hands-on care and support for patients and their families in the comfort of our patient’s home. As well as nurses, the Westlake Village interdisciplinary team also includes Hospice Physicians. A hospice physician is responsible for providing care during the last few weeks or months of a patient’s life. It is a physician’s duty to make sure their patients are as comfortable as possible, while making recommendations for future care. Hospice Physicians are flexible and responsive. They are able to see patients as often as needed to provide direction and to ease suffering.
A person who has chosen hospice care has chosen to accept the end of life. But just because you’ve made this choice doesn’t make it easy. Between managing your physical symptoms and maintaining mental stability and peace of mind, the end of life period can be very challenging. At Westlake Village Hospice, we’re here to help. First, by choosing hospice care, you’ve gained access to a staff of nurses and physicians whose job is to assist you in any way possible. Hospice nurses are trained in fields such as geriatrics, pharmacology, psychology, and medical ethics, all skills that they can call upon in case of emergency. As your condition progresses, your doctors will respond with the appropriate treatments.
